When the practice starts running you

You are good at this. You keep up with continuing education, you care about your patients, and you know your clinical skills are real. That is exactly why the gap is so frustrating. You can feel that you could be doing better dentistry, planning more comprehensively, and growing the practice, and yet most weeks you are simply trying to stay above water.

A lot of doctors describe the same quiet thoughts. The team is not fully aligned. Case acceptance is softer than it could be. Larger cases feel risky enough that it is easier to keep them small. The schedule is full, the work is hard, and somehow it still feels chaotic. You know what you want the practice to become, and you are not sure how to get there from inside the daily noise.

There is a name for that pattern. The signature of mediocrity is chronic inconsistency, the kind where you are reacting all day and nobody quite knows what to expect. Working harder rarely solves it. I understand this from the inside, because I lived it. Early in my career I was a capable clinician and a somewhat green leader. Leadership is something we don’t learn in dental school. After many courses and books there is a simple concept from Brené Brown that I now repeat constantly: communication is kindness, and clarity is kindness. Having trained clinically through the Kois Center and spending two decades doing comprehensive, cosmetic, and full-mouth rehabilitation dentistry, I then co-founded a group practice that grew to thirty-nine locations and five hundred team members. AND none of that matters unless we translate it into something you can use on Monday morning.

The doctors I work with tend to move along the same arc. They go from reactive to intentional, from hesitant to confident, from chaotic to consistent, and from feeling isolated to feeling supported. The dentistry gets bigger and gets better. The team gets more engaged. The workdays become more enjoyable all because the practice finally feels like the one you pictured when you started.

The opposite of growth is stagnation, and I believe when we aren’t pushing ourselves we actually go backwards. Energy is always moving- we pick the direction it flows. Our brains often default to what is familiar over what is healthy, which is why standing still can feel safe even when it is quietly working against you. The familiar version of the practice is comfortable, and over time the opportunity cost is the better practice you were capable of building.
